The Community Health Worker (CHW) will fully integrate the multidisciplinary care team of Meharry Community Wellness Center (MCWC). He/she will develop a close relationship with patients, coach them and facilitate access to services. He/she will identify, track, re-engage and monitor clients lost to care. The CHW will assist clients in dealing with competing priorities, adherence with medication, clinic visits, and specialty referrals, linkages with community-based organizations, wellness education, harm reduction, and navigation through the healthcare delivery system. The CHW will meet with clients in the clinic, at their homes, and different access points in the community. Daily Operations include adherence counseling, harm reduction, health promotion, and patient navigation; collecting all required data elements and providing timely reports; outreach services to persons living with HIV through direct personal contact and collaborative community partnerships; linkages with Meharry Community Wellness Center, community-based organizations and specialty services; attending all mandatory meetings; and performing other related duties as assigned by Supervisor or Executive Director. Required skills include basic knowledge of HIV/AIDS, being a team player and well-organized, skills in engaging people living with HIV, ability and willingness to use both knowledge and personal experience in facilitating linkage of individuals and groups to essential HIV health related services, communication skills and proven behaviors consistent with maintenance of health and wellness, ability to establish trust based relationships with people in an appropriate and ethical manner, knowledge and ability to access community resources, ability to attend mandatory staff meetings, basic computer skills and ability to collect data elements and generate reports in a timely manner, and motivation to serve people with HIV and get trained as assigned by Executive Director or supervisor. Required education and experience include a high school diploma with commensurate experience; associate or bachelor degree preferred, familiarity with people living with HIV is mandatory, familiarity with Nashville/Davidson County, familiarity with Ryan White programs and Meharry Community Wellness Center is highly desirable, must possess valid driver’s license, and willingness to attend all required trainings.
