About the Role:

As a Direct Support Professional at HealthChoice Waiver Services , you will play a vital role in empowering individuals with disabilities, developmental challenges, or special needs to lead independent and fulfilling lives. You will provide hands-on support, encouragement, and assistance in daily living activities while fostering a safe, respectful, and engaging environment.

Key Responsibilities:

• Assist individuals with daily living tasks , including personal care, hygiene, meal preparation, and mobility support.

• Provide emotional support and companionship while promoting independence and self-confidence.

• Support individuals in community integration , such as outings, social events, and recreational activities.

• Help develop and implement individualized care plans to achieve personal goals and improve quality of life.

• Document daily activities, progress, and any concerns to ensure the highest level of care.

• Communicate effectively with team members, families, and healthcare professionals.

Qualifications & Skills:

• Experience in caregiving, healthcare, or social services is a plus but not required—training is provided!

• Compassionate, patient, and dedicated to making a positive impact.

• Ability to lift, assist, or provide physical support as needed.

• Strong communication and teamwork skills.

• Valid driver’s license.
